Once your inquiry is submitted, our Outpatient Admissions Team will review your information, request any needed medical records, and guide you through your options.
New to Craig Hospital (New‑to‑System Patients)

Patients with a Brain Injury (BI)
If you have a brain injury and are seeking therapy, evaluation, or specialty medical clinic services, please complete the BI Outpatient Inquiry Form.
Patients with a Spinal Cord Injury (SCI)
If you have a spinal cord injury and are interested in outpatient therapy, evaluation, or specialty medical clinic services, please complete the SCI Outpatient Inquiry Form.
After You Submit Your Inquiry

- You will be contacted within 7 business days of receiving your inquiry.

Former Craig Hospital Patients
If you have been to Craig Hospital before — either for inpatient or outpatient services — you may be eligible to return for outpatient therapy and/or specialty medical clinic services.
How to Schedule as a Returning Patient
Please call Outpatient Admissions directly at 303‑789‑8035, as you normally do, to discuss your needs.
If you have not been seen at Craig in three or more years, we may request updated medical records prior to scheduling.

Our Outpatient Services
Spinal Cord Injury Outpatient Program
Patients with spinal cord injury (SCI) require specialized follow-up for maximum health, prevention of complications, and long term management of costs. Craig’s outpatient SCI rehab program provides that specialized care with extraordinary patient outcomes.
Brain Injury Outpatient Program
Many brain injury (BI) patients continue their recovery after discharge from an inpatient BI rehab program. Craig’s BI outpatient therapy program is individually designed and delivered for each patient based on their medical, functional and clinical needs.
Outpatient Medical Clinic
Craig’s Outpatient Medical Clinic supports spinal cord injury and brain injury patients who are transitioning from inpatient to outpatient status, as well as outpatients returning for Interdisciplinary Outpatient Evaluations (IOEs), formerly known as re-evaluations.
