The Federally Required Transparency Data is based on information gathered from claims and insurance payment files. It is not a guarantee of what you will be charged or what you will owe. Your actual charges will be based on the care actually provided. Actual charges may differ from the information in the Federally Required Transparency Data for many reasons, including but not limited to the seriousness of your injury, the duration of your care, the services actually received, any complications you may have, and additional tests or procedures your providers order. If you have insurance, your insurance benefits will ultimately determine the amount you owe to the facility (including deductibles, co-pay, co-insurance, and out-of-pocket maximums).
The Federal Price Transparency Data only includes the fees charged by providers who are employed by Craig Hospital. Physicians fees, including but not limited to attending physicians, radiologists, internal medicine and other consultants are not included in this data. Durable Medical Equipment and transportation (air or ground) are also excluded from these estimates.
Craig Hospital is not liable for any difference between the information contained in the Federal Price Transparency Data and your bills.
