What is MyChart?
MyChart is an online portal used to obtain your medications, test results, upcoming appointments, medical bills, price estimates and more all in one place, even if you’ve been seen at multiple health care organizations. Please use a supported browser such as Chrome or Edge for the best experience with MyChart.
Any requests for help with the MyChart portal can be directed to CommonSpirit’s MyChart Support Service line at 866.414.1562.
Craig Hospital, as a Community Connect partner, uses the CommonSpirit Health (formerly Centura Health) platform for MyChart. Craig is not a CommonSpirit Health facility and maintains status as an independent not-for-profit hospital.
Access MyChartHow Do I Sign Up?
There are several ways to sign up for your MyCommonSpirit – Mountain (MyChart) account:
- Clinic staff can directly send you an activation message while you’re at the clinic front desk or in a session.
- You will receive a text message or email with an activation link when an appointment is scheduled and when you check in for an appointment.
- If you receive an activation code on your After Visit Summary or on a billing statement, you can set up your new account here.
- You can sign up online at MyCommonSpirit – Mountain account here by matching your information against what is on file in your medical record or with third-party identity verification.
- If you would like to set up a proxy in MyChart, click here then click on the “Granting friends and family access to your online record” drop down on the MyCommonSpirit page.
- Medical records are available immediately.
- You can share records with providers and family members.
- Medication lists are available for patients to see and share.
- You can link with other MyChart facilities so all of your charts are in one place.
Access Medical Records
If you have been a patient at Craig Hospital, you may request a copy of your medical record, radiology images and billing records for yourself – or have them sent to a third party – upon written authorization from the patient or guardian.
To authorize the release of your records, please download and print the Patient Request for Medical Records form or the Third Party Request for Medical Records form, available as a fillable PDFs in English and Spanish versions below.

- Fax the form to 303.789.8495, or
- Mail the form to the address below, or
