Patients can expect an interdisciplinary team care model for BI outpatient rehabilitation. This interdisciplinary team strives to maximize physical, cognitive, behavioral and emotional functioning.

Each team member conducts an initial evaluation with the patient. Together, the patient, family and clinician establish an individualized treatment plan with appropriate goals for the patient.
Treatment Plan
A treatment plan is built on an understanding of the patient’s neurological diagnosis and prognosis, as well as the relationships between the injury and behavior. The team, including the patient and family, continually review and adapt the treatment plan within the context of the patient’s personality, goals, resources, environment, and the patient’s community.
Interdisciplinary Outpatient Evaluations (IOE) and New Patient Evaluations
When warranted and appropriate, previous Craig Hospital inpatients and outpatients may return to Craig Hospital for an Interdisciplinary Outpatient Evaluation (IOE) in response to specific questions or needs that may have arisen, such as medical changes, improvements or declines in function, aging of family caregivers, need for new equipment, etc.
Evaluations for outpatients with a brain injury who are new to Craig Hospital are considered on a case-by-case basis. Typically, such evaluations are considered for patients who require evaluations by all members of the interdisciplinary team and have a diagnosis of moderate to severe acute brain injury.
