Clinical Care Management

Clinical care managers (CCM) are critical members of each interdisciplinary team. CCMs provide assistance in patient and family counseling, education, accessing benefits, team coordination, planning for home, and advocacy.

Meet Our Inpatient Clinical Care Management Team

  • Kent Hamstra Director of Clinical Care Management

    Clinical Care Management

    Kent Hamstra MA, received his training and degree in Psychology from Hastings College in 1985 and Master of Arts degree in Rehabilitation Counseling from University of Iowa in 1986. He has been a member of the Craig Hospital inpatient TBI clinical team since 1987. Kent has been involved with multiple projects involving TBI. He is a member of the Craig Hospital TBI Supervisors Group.

  • Nichola Abeyta Inpatient Clinical Care Manager

    Clinical Care Management

    Nichola Abeyta, MEd, started at Craig Hospital in 1989. She has a Master’s degree in Education and BA in Social Work. Niki is a member of the Spinal Cord Injury Program Committee Peer Mentor Committee and a team leader for the Motivational Interviewing initiative. Niki also assists with new employee orientation. She represents Craig Hospital Patient and Family Services as a lead clinician for a multi-year, multi-facility study on spinal cord injury outcomes. Niki has co-authored two published study articles.

  • Ruth Grenoble Inpatient Counselor

    Ruth Grenoble, LCSW, works in the inpatient Clinical Care Management department for the spinal cord injury team. Ruth earned her Bachelor’s degree in Philosophy from Colorado College in 2008 and her Master’s in Social Work from University of Chicago in 2010. Ruth joined Craig Hospital in 2016 after working for the Department of Veterans Affairs (VA) in several locations, including the Spinal Cord Injury Center in Hines, IL, and the Acute Rehab and Subacute Rehab units in Denver, CO. Ruth has a particular interest in the intersection between Social Work theory and practice, and is a passionate advocate for bringing philosophical and ethical discussions into clinical practice in medical settings.

  • Avery LaFleur Inpatient Clinical Care Manager

    Avery La Fleur, MSW, has been a part of the Craig Hospital family since 2004. After completing his undergraduate degrees at Wartburg College in Waverly, Ia., he moved to Colorado. After several years working in corrections, Avery went on to get his master's degree in social work at the University of Denver Graduate School of Social Work, where his emphasis was in medical social work as well as individual, family and group counseling. He is also a guest professor for the University of Phoenix. Avery provides educational classes at Craig Hospital in the areas of sexuality and romance, discharge planning, and social skills.

  • Karen Leonard Inpatient Clinical Care Manager

    Karen Leonard, MSW, works in TBI inpatient Clinical Care Management. Karen received her B.A. degree in Psychology from Colorado State University in 1997 and her Masters in Social Work from University of Denver in 2001. Karen has worked in the Rehabilitation Social Work field for the past 16 years. Karen joined Craig Hospital in 2015 as a counselor in the Clinical Care Management Department. Karen enjoys spending time with her family outside of work.

  • Kent Rosemberg Inpatient Clinical Care Manager

    Kent Rosemberg, MSW, has been a part of Craig Hospital since 2012. She completed her undergraduate studies in Sociology at Fort Lewis College in Durango Colorado. After returning home to Denver, she spent 3 years working at a residential facility in Aurora for at-risk adolescents. Kent went on to get her Masters degree in Social Work, at the University of Denver, Graduate School of Social Work, Denver, Co. where her emphasis was in medical social work and interpersonal trauma studies. She now provides counseling at Craig Hospital to patients and families who have sustained traumatic brain injuries.

  • Carol E. Wineinger Inpatient Clinical Care Manager

    Clinical Care Management

    Carol E. Wineinger, MA, LPC, joined Craig Hospital in 2006, and works with inpatients who have spinal cord injuries and/or combination brain and spinal cord injuries. Carol's education includes a bachelor's degree in psychology from the University of Nebraska and a master's degree in counseling psychology from the University of Northern Colorado. She is a Licensed Professional Counselor for the State of Colorado, and an active member of The Academy of Spinal Cord Injury Professionals.

  • Becki Bushnell Outpatient Counselor

    Becki Bushnell, M.S.W., received her B.S. degree in Social Work from Metropolitan State College of Denver in 2005 and her M.S.W. in Social Work from the University of Denver in 2008. Becki joined Craig Hospital in 2008 as a Outpatient Counselor in the Patient and Family Services Department. In 2008, Becki received her certificate in Animal Assisted Therapy and became an Interdisciplinary team member of the outpatient skin clinic.

  • Lenore Hawley Brain Injury Education Counselor

    Clinical Care Management

    Lenore Hawley, MSSW, LCSW, CBIST, has worked with individuals with TBI for more than 30 years providing clinical services, program development, advocacy and research. She completed advanced training as a Certified Brain Injury Specialist Trainer (CBIST), and is the author of A Family Guide to the Head Injured Adult, Self Advocacy for Independent Life, and co-author of Group Interactive Structured Treatment-GIST: for Social Competence. Ms. Hawley is currently Co-Principal Investigator for a RMRBIS study on psychological well-being, Principal Investigator for the Self Advocacy study, and serves as Co-Investigator and Co-Director of Intervention, Training and Dissemination on a Department of Defense multi-site grant on social competence. In addition, she provides group therapy for civilians with TBI, and for soldiers and veterans with TBI and PTSD through her private practice.

  • Kathy Hulse Outpatient Counselor

    Kathy Hulse, LCSW, CCM is an outpatient counselor with Clinical Care Management.  She received her MSW from Virginia Commonwealth University in 1988.  She joined the outpatient clinic at Craig Hospital in 1989.  Kathy helped to develop the Medicaid Buy-In Program for working adults with disabilities for the state of Colorado.  She is a member of the Academy of Spinal Cord Injury Professionals and the Society for Social Work Leadership in Health Care.  She serves on the board of directors for the Colorado Disability Benefits Support (DBS) Program.

  • Victor Towle Outpatient Counselor

    Clinical Care Management

    Victor Towle, M.S., is an Outpatient Counselor with Patient and Family Services working with the TBI team. He joined Craig in 1997. He has a BA in Psychology and an MS in Rehabilitation Counseling from SIU-Carbondale. He has served on the Boards of Brain Injury Association of CO and Rocky Mountain Brain Injury Association and several Community Boards. Victor has also worked with the CO Brian Injury Task Force.
