Our Physicians (CNS Medical Group)

If you are a physician anywhere in the U.S. and you want to refer a new patient to Craig for inpatient rehabilitation or outpatient evaluation, ask your discharge planner or office staff to call our Admissions Department directly at 303-789-8344. If you would like to discuss your referral with one of our Craig doctors, we encourage you to call our CNS Medical Group at 303-789-8220 and ask for the On-Call Referral physician. One of our doctors will speak with you personally, take the referral information, discuss the patient’s condition, and facilitate the referral process with our Admissions Department. We recommend that the referral be called in as soon as possible to expedite admission. If you would like to receive physician reports from Craig during the patient’s treatment at Craig, please obtain the necessary authorization from the patient and give your mailing address to the CNS Medical Group or our Admissions Department. Request that your information appear on the patient’s Craig “Face Sheet.”
